SafeHaven Hospice works with a Case Manager who oversees the care plan, adjusts the plan as needed, and consults with the family, patient, and the patient’s doctor during the transition.

The SafeHaven Case Manager is also there for the family to help before, during, and after the transition. The Case Manager is your rock, advocate, and companion during all phases of the journey.
